Dog Adoption Application Form

Thank you for your interest in adopting a dog! Our adoption applicants are screened for suitability and compatibility as well as criteria related to care and commitment. We take the time to carefully read each and every application. If we find your application aligns well with the dog you have applied for we will contact you within a couple of days to further the adoption process. Unfortunately we are unable to contact all applicants. Only applications that align with the dog’s lifestyle, physical and medical needs will be contacted. We reserve the right to deny an adoption applicant.

Our adoption fee is currently $525.

Please note that we only adopt to residents of Ontario.

If you have trouble with the reCAPTCHA at the bottom of the form when submitting, these are the common causes:

Your browser is not updated to the latest version – One of the most common reasons why this error occurs is due to an outdated browser version (e.g. Chrome, Internet Explorer, Firefox, etc.). reCaptcha will actively look at the browser version before allowing you access. If this is the case, the solution is to update your browser to the latest version.
VPN or Proxy service is not playing well with reCaptcha – Try turning off VPN and Proxy.
Network issue – rebooting modem/router should help to resolve it. Please also clear the browser data and cache when rebooting modem/router.
Clear cache and cookies and try using an incognito browser tab.

Are you aware that there is an adoption fee of $525? This includes at minimum: a veterinary exam, spay/neuter, the core vaccinations (defined by the OVMA) against Rabies, Parvovirus, Distemper and Adenovirus-2, a microchip implant and tests for heartworm and tick-borne diseases. Many of our rescued dogs also have full blood panels run, dental surgeries and other necessary procedures as per their specific needs. *

Now that COVID-19 restrictions have been lifted we are able to conduct in-home visits again. An in-home visit involves one of our volunteers visiting in person to check the indoor and outdoor living space for the dog you wish to adopt. During COVID, we modified our home visit procedure by offering a virtual tour using Skype/Facetime, video taken by the potential adopter or, in some cases, a gallery of photos could be submitted. (Please note any videos or photos submitted will not be shared outside our organization). Virtual home visits remain an option for those who are not comfortable with in person visits, however, in person visits are preferred. Would you consent to a home visit at this time? *
